1. Covering letter addressed to Joint Director of Agriculture (Fertilizer & Plant Protection), Head office, Bengaluru

2. Debiting of required fee of Rs. 2,000/- for each grade in the Khazane-2 (Challan)

3. Bank Guarantee for Rs. 50,000/-

4. Declaration of Laboratory facilities and Equipment’s as per Cl.21A of FCO, 1985

5. Certificate for Nomination as Responsible person

6. Detailed bio-data of the Responsible person (Name, Age, Qualification, Designation, Present & Permanent address, Phone Number-Office& Mobile, Experience etc.,)

7. Qualified person Degree certificate and Marks Card [B.Sc.(Agri) or B.Sc. with Chemistry as optional subject

8. Qualified person Appointment letter

9. Acceptance of the appointment order by the Chemist/Qualified person

10. Registration certificate from Industries & Commerce Department (SSI Certificate)/Certificate of Incorporation as per Companies Act/Certificate from District Industries and Commerce

11. Udyog Aadhar Certificate

12. Copy of Wholesale Fertilizer Dealer license

13. Firm Registration certificate

14. G.S.T. Registration Certificate & Assessment

15. Affidavit copy of Declaration regarding Responsible person as per Cl.24 of F.C.O. 1985

16. Enclose copy of Aadhar card of Responsible person
