Eligibility: Any Person who is holding valid licenses issued under Part VI of the Drugs and Cosmetics Rules, 1945.

1.Covering letter

2.Qualified persons proforma and or Competent persons proforma

3.Address proof of CP

4.CERTIFICATES AND PHOTO OF CP

5.Reliving letter of QP/CP if he was working in other firm.

6.Letter of previous Licensing Authority for having deleted the name of QP/CP, if he was working in other firm.

7.Existing licenses

8.Xerox Copy of licenses CP Last Approval letter

9.Registered Pharmacists / Competent person affidavit

10.Biomedical Waste disposal Affidavit/Under taking (if not given already)
