2.Registration Certificate issued by Central Insecticide Board & Registration Committee

3.Allotment letter/Rent agreement/ Lease agreement copy of the building (as applicable)

4.Building Plan of the Unit

5.List of Analytical Instrument in the Laboratory

6.Infrastructure facilities available in the unit

7.Local qualified doctor’s acceptance letter for periodical health examination & during emergency

8.List of first aid measures adopted

9.Karnataka State Pollution Control Board Consent Letter

10.Municipal/ Panchayat license

11.Industries Dept. License (S. S. I.)

12.Memorandum and articles of Association/ Partnership deed/affidavit of Proprietorship on Rs. 100/- stamp paper and notarized

13.List of Directors/ Partners/Proprietor with age, fathers name and complete address

14.Appointment Order of chemist and expert staff

15.Acceptance Letter of chemist and expert staff

16.Marks cards and Degree certificates of chemists and expert staff

17.Affidavit with age, fathers name and complete address of chemists and expert staff and responsibility the quality control of the products manufactured on a Rs. 100/- stamp paper notarized

18.Safety measures undertaken in the unit

19.List of facilities extended to the workers in the unit (Wash rooms, change rooms, toilets etc.)

20.Specimen Signature 21.Passport size Photgraph 22.Authorization Letter/Nomination Letter

Service Charge (Free for Online Submission) : Rs 100
