Eligibility: The agency should have a valid ASU number

Supporting Document:

1) Cover letter

2) Affidavit from Magistrate (not below Grade I) on Rs. 50 Stamp paper

3) GMP certificate

4) Approved Product List

5) Others

Application Fee : Rs 250

Service Charge (Free for Online Submission) : Rs 100

Delivery Time (Days) : 30

Procedure for applying:

1.Application submission (Online, B1/K1 centres, CSC centres, AJSK centres, BapujiSevaKendras)

2.The application is routed to the First Division Assistant (FDA)

3.Verification by the First Division Assistant (FDA). Recommendations of the First Division Assistant (FDA) are sent to the Assistant Drug Controller/ Drug Inspectorfor review

4.Verification by the Assistant Drug Controller. Recommendations of the Assistant Drug Controller are sent to the Deputy Drug Controller for review

5.Verification by the Deputy Drug Controller. Recommendations of the Deputy Drug Controller are sent to the Drug Controller for review

6.Review by the Drug Controller. Approve or reject the application request and forward it to Deputy Drug Controller

7.Deputy Drug Controller to sign and issue the certificate or endorsement
