7. AFFIDAVITS ON BIOMEDICAL WASTE DISPOSAL AFFIDAVIT UNDER TAKING(IF NOT GIVEN ALREADY)

8. XEROX COPY OF LICENSES LAST RENEWAL FORM 21C QP OR CP LAST APPROVAL LETTER
